Every figure is per-NPI and per-CPT, built from federal Transparency-in-Coverage filings — a scope of 314M+ rate records across 500+ payers and all 50 states — compared to your LOCAL peers. We target the top of your local market — the 90th percentile (P90) — not the median: asking at the median concedes it before the payer responds, and annual escalators then compound downward from there. P90 is where a 60-expert war-room said you should aim to land — your documented negotiation target, not a guarantee. Each line is traceable to a specific payer's published filing, so it's defensible in an actual rate inquiry. No national averages, and no PHI — it's all published / contracted rate data, not patient records.
